We built this resource because we kept seeing the same gaps - guides that covered licensing timelines but ignored the tax math. Guides that mentioned "great schools" with no names or numbers. Guides with zero intel on where a physician's spouse would actually want to live, eat, and relax.
This guide fills every one of those gaps - written with the same precision you bring to your work: data-driven, evidence-based, and organized for fast comprehension. Welcome to Central Florida. Let's get you home.
Twenty-nine chapters, one move handled.
Why Central Florida?
Central Florida is not the market most physicians picture when they hear "Orlando." The metro has become one of the most dynamic healthcare ecosystems in the country - anchored by a purpose-built Medical City at Lake Nona, the largest non-government hospital in the nation at AdventHealth Orlando, a Level I Trauma Center at ORMC drawing patients from across the Caribbean and Latin America, and a UCF-HCA partnership redefining academic-community medicine.
The greater Orlando metro now exceeds 3.5 million residents and keeps growing faster than nearly every major U.S. metro. That growth translates directly into physician demand across virtually every specialty. For the doctor evaluating markets, Central Florida checks the clinical, financial, and lifestyle boxes simultaneously.
"It checks the clinical, financial, and lifestyle boxes - at the same time."
The tax math: what zero state income tax really means.
Florida is one of a handful of states with no state income tax - and for a high-earning physician, that is not a rounding error. It is the single largest recurring financial difference between practicing here and practicing in California, New York, or New Jersey.
On a physician's income, the gap between a top state rate and Florida's zero compounds every year you practice - money that stays in your household rather than leaving it. Pair that with Florida's homestead protections and the Save Our Homes assessment cap, and the long-run math favors Central Florida decisively.
The chapters ahead show how to establish domicile correctly the first time - and survive a former state's residency audit if it comes.

Talk to Sean & BarbEstablishing domicile - correctly, the first time.
Moving to Florida physically is not the same as establishing Florida legal domicile - and that distinction is aggressively enforced by tax authorities in New York, California, and New Jersey. Do this wrong and you can owe back taxes, penalties, and interest on your full state income-tax liability for years you thought you'd escaped.
The protocol below is what we walk every relocating physician through. Treat it as something you complete in your first weeks, not a someday list.
- Purchase or lease a Florida home as your primary and principal residence.
- File a Declaration of Domicile with your county clerk (Orange, Seminole, or Osceola).
- Obtain a Florida driver's license within 30 days of establishing residency.
- Register all vehicles in Florida and surrender prior-state plates.
- Register to vote in Florida and close prior-state voter registration.
- File for Homestead Exemption by March 1 of the applicable tax year.
- Change your address with banks, brokerages, the IRS, FINRA, and all professional boards.
- Update estate documents - will, revocable trust, POA, healthcare surrogate - to Florida law.
- Spend the majority of your time in Florida; document it with calendar, card, SunPass, and phone-location records.
- Join Florida organizations - professional, medical, religious, social, and club.
- Move meaningful personal property - artwork, heirlooms, valuables, primary vehicle.
- Resign from clubs, memberships, and boards in your prior state.
Florida licensing: three paths, one goal - practice ASAP.
Path 1 - Interstate Medical Licensure Compact (IMLC). Available in 43 states plus DC and Guam, the IMLC is almost always the fastest route for qualified physicians. You'll need an active, unrestricted license in an IMLC member State of Principal License, USMLE or COMLEX as your exam path, no disciplinary history, board certification (or completed ACGME/AOA training), and at least two years of active practice in the past four.
| Step | Estimated timeline |
|---|---|
| Application + document submission | 1-2 weeks |
| Background check / fingerprints | 7-14 days |
| Letter of Qualification (LOQ) issued | Avg. 37 days; 52% in ≤30 days |
| Florida license issued after LOQ | Avg. 20 days; 51% in ≤7 days |
| Total (best case / typical) | 3 weeks / 4-8 weeks |
| IMLC fee | $700 · imlcc.org |
Path 2 - Standard Florida license by endorsement, for physicians not using the IMLC. Florida processes its own license separately even with an IMLC Letter of Qualification, so start the Florida DOH application simultaneously - don't wait for the LOQ.
| Stage | Timeline |
|---|---|
| Application submitted to DOH | Day 1 |
| Application posted on DOH system | 7-10 days |
| Review initiated | Within 30 days of posting |
| Primary source verification | 4-12 weeks |
| License issued (MOBILE Act standard) | 7 days once complete |
| Total realistic timeline | 2-6 months |
Path 3 - Telemedicine temporary certificate, useful for maintaining consulting income or academic collaboration while your full application processes.

The physician compliance checklist.
Your master moving checklist - every item time-sensitive. Tick them off as you go; your progress is saved to your edition.
Credentialing & payer enrollment - start before you land.
Most physicians underestimate how long credentialing and payer enrollment take - and it's the number-one reason for a gap between your start date and your first paycheck. Build the timeline backward from day one.
| Process | Typical timeline | Notes |
|---|---|---|
| Hospital credentialing | 60-120 days | Cannot see hospital patients until complete |
| Medicare enrollment | 45-90 days | Apply via PECOS (pecos.cms.hhs.gov) |
| Medicaid enrollment | 30-90 days | Apply via the Florida AHCA portal |
| BCBS / Aetna / United | 60-90 days each | Submit all simultaneously |
| CAQH ProView | 2-3 weeks | Complete immediately - every payer requires it |
Physician mortgage loans.
For a physician buying in Central Florida's $1M+ luxury market, a physician mortgage loan is almost always the optimal financing structure - built around a doctor's income trajectory rather than a conventional borrower's.
| Feature | Physician loan | Conventional jumbo |
|---|---|---|
| Down payment | 0-5% on $1M+ homes | 10-20% typical |
| PMI | None | Required under 20% down |
| Student-loan treatment | Excluded from DTI | Counted fully |
| Pre-employment close | Yes - with signed contract | No |
| Max loan amount | Up to $2.5M (select lenders) | Conforming: $832,751 |
Physician lenders active in Central Florida include First Horizon (up to $2.5M, 0% down for attendings - strong for jumbo luxury), TD Bank (residents, fellows, and attendings; can close roughly 90 days pre-start), Fifth Third (flexible DTI, competitive rates), and SouthState Bank (deep local presence). We'll connect you with the right desk for your stage and price point.
Homestead exemption & the Save Our Homes cap.
Once you purchase and establish primary residency, Florida's homestead system becomes one of your most powerful ongoing financial tools - compounding in value every year you own the home.
The $50,000 exemption reduces your home's taxable assessed value by up to $50,000, saving roughly $430-$750 per year depending on county millage. Save Our Homes then caps your assessed-value increase at the lower of 3% or CPI annually - no matter what the market does. In an appreciating market, that produces enormous compounding savings.
| Year | Market value | Assessed (SOH) | Tax difference |
|---|---|---|---|
| Year 1 | $2,500,000 | $2,450,000 | - |
| Year 5 | $3,200,000 | $2,810,000 | ~$28,000 saved |
| Year 10 | $4,000,000 | $3,251,000 | ~$60,000 saved |
Portability: if you previously owned a homesteaded Florida property, you can transfer up to $500,000 in accumulated Save Our Homes savings to your new home. Deadline: file by March 1 of the tax year at your county property appraiser's website.
The first 90 days: your launch sequence.

Seminole County Public Schools (Lake Mary, Longwood, Oviedo) is the #1 district in Central Florida and ranks second among Florida's fourteen largest.
